1. Your FSA ID Create FSA ID

2. Your Social Security Number or Alien Registration Number

3. Parent Information: Date of Birth, SSN, Married/Divorce/Single, if married, wedding date, 2021 Income Tax Information, W2s

4. Parent must have an FSA ID as well
